Information Update home address or phone numbers. Use this form to change the current address and home phone we have on file for you, and to request changes on your membership.
- Account Nickname Change Member described nickname. Change authorization form to request a change or add a nickname to an existing Member Described Account.

Stop Payment Stop payment order of a check or ACH (pre-authorized electronic withdrawal) when it is presented to us for payment if it has not already cleared your account. Stop payment orders cannot be placed on DCU Visa Credit Card, Check Card, or ATM Card purchases.

Visa / Check Card / ATM Card Dispute a Visa, Check Card or ATM Transaction. Use this form to report a transaction that you participated in, but are disputing with the merchant.

Visa / Check Card / ATM Card Report a fraudulent or unauthorized Visa or Check Card Transaction. Use this form to report any DCU Visa, DCU Visa Check Card or DCU ATM Card transaction that you did not make or you feel is fraudulent.

Visa / Check Card / ATM Card Order a Replacement Card. Order a replacement plastic card for a DCU Visa Credit Card, DCU Visa Check Card, or a DCU ATM Card that has stopped working.

PC Branch Cross Account Transfers Use this form to request the ability to transfer funds to another DCU member.

Certificate Certificate Change Authorization Use this form to add-on funds, withdraw funds, transfer funds, or change your term at maturity on your DCU Savings Certificate.

Mail Branch Reorder Mail Branch Envelopes Use this form to order postage-paid envelopes that include a built-in deposit slip if you need to send deposits to DCU by mail.
