- Visa Credit Card Application(.pdf format) – Use to apply for a DCU Visa if you don't want to apply online.
- Visa Authorized User Application (.pdf format) – Use to give other individuals access to your Visa account when they are not co-borrowers.
- Loan Payment Method Authorization (.pdf format) – Use to have DCU set up electronic loan payments by transfer from your DCU savings or checking account. Note: you can set this up on your own with PC Branch Automatic Transfers.

- Visa Classic, Gold, and Platinum Disclosure (.pdf format, 11 pages) – This covers disclosures related to DCU's Visa credit card programs.
- Visa Credit Card Additional Federal Disclosure Table and Schedule of Fees (.pdf format) – Provides current Annual Percentage Rate, fee, and grace period information.
